Após Jason Bourne revelar publicamente o projeto Treadstone, Eric Byer (Edward Norton) é encarregado de apagar os rastros que possam incriminar o governo dos Estados Unidos neste e em outros projetos sigilosos. Um deles chama-se Outcome, um projeto que pretende suprimir a dor e aumentar a sensibilidade, inteligência e força de seus agentes através de remédios tomados periodicamente. Com o fim do Outcome, seus agente passam a ser eliminados a mando do próprio governo. Aaron Cross (Jeremy Renner) é um deles, mas consegue escapar sem que Byer perceba. Em busca de respostas, ele vai à casa da doutora Martha Shearing (Rachel Weisz) e a salva da morte certa. Juntos, eles precisam encontrar um meio de sobreviver ao mesmo tempo em que Aaron, sem seus remédios habituais, começa a sentir os efeitos colaterais da abstinência forçada.
