PREFACE TO THE FIRST EDITION
The purpose of this book is to present within one volume a concise but complete discussion of the fundamental aspects of pain, the various diseases and disorders in which pain constitutes a major problem, and the methods employed in its management, with special emphasis on the use of analgesic block as an aid in the diagnosis, prognosis, and therapy. Although several books dealing with certain phases of this problem are available, none is complete from the standpoint of the practitioner; for it is necessary for him to consult several texts in order to obtain information regarding the cause, characteristics, mechanisms, effects, diagnosis, and therapy of pain and management of its intractable variety with analgesic block and certain adjuvant methods. The present volume is the product of the author\'s desire to facilitate the task of the busy practitioner and to supply him easily accessible information with the conviction that this will induce more clinicians to employ these methods of diagnosis and therapy.
The expansion and diversification of orthopaedic surgical procedures, however, also demands an ever greater technical expertise from the orthopaedic surgeon. Many of the several thousand procedures current in orthopaedic surgery are difficult and complex, and their success or failure depends very much on the orthopaedic surgeon\'s mastery of the subtleties of technique.
This book has been developed to meet the need for a text that stresses the technical details that the orthopaedic surgeon must know to perform these intricate surgical procedures well. The emphasis throughout is on the details essential to rapid, smooth, complication-free surgery. Each contributor has written a tightly focused chapter, in which he describes his preferred method of performing specific operations—including hints and â€کfine points’ that have proven of value in his own experience. A particularly important feature of each chapter is the discussion of how to avoid pitfalls and manage complications.
The table of contents, index, and detailed outline of headings at the beginning of each chapter should facilitate rapid and easy location of any procedure. Because no book can describe in detail every orthopaedic surgical procedure, alternative methods to those discussed in the text are noted and cited in the comprehensive reference lists appearing at the end of each chapter. Historical aspects, reviews of the literature, and compilations of results are mentioned only in passing, although necessary data are provided to justify the recommendation of the specific procedures described.
The text has been organized with the aim of making it as useful as possible to the practicing orthopaedic surgeon. The many operative procedures included are arranged partly according to anatomic region, partly according to surgical modality, and partly according to type of injury or disease—depending on the context in which a particular procedure is most likely to be considered by the orthopaedic surgeon. The eight chapters in Section I, Surgical Principles and Techniques, provide a unifying overview of the general principles important to successful surgery—particularly preoperative planning, a neglected topic that is critical to success. Each of the fourteen major sections of the text that follow begins with several chapters on general principles that introduce the chapters on specific operations. The technical expertise and experience represented in these chapters is the foundation of this book.
